It’s Good to Be Home

Ohio’s PASSPORT Medicaid Waiver program offers an alternative to nursing home placement through customized care plans that help older adults maintain a safe, happy life at home.

This service also eases caregiver responsibilities and guides you toward a more positive future for yourself and your loved one.

Eligibility Requirements

60 or older

Have qualifying functional impairment

(need assistance with bathing, meals, etc.)

Medicaid-waiver eligible

(low-income household)

The Process

1.

Contact Direction Home of Eastern Ohio to complete a screening over the phone and schedule a free in-home assessment.

2.

We will determine preliminary eligibility and care needs and will provide information about various long-term care options.

3.

If eligible, a dedicated care manager will help develop a customized care plan and coordinate the services you need. *

4.

Over time, the care manager will monitor services for quality and make changes to the care plan as necessary.

*If not eligible, we will assist you in finding alternative programs to get you the help you need.
