NOTICE 0924156: Public Comment Period for PASSPORT Medicaid Waiver Renewal
Ohio has posted for public comment on the proposed amendments to the Assisted Living and PASSPORT Medicaid Waivers, prior to the submission to the Centers for Medicare and Medicaid (CMS) for review. The period for public comment on the proposed PASSPORT Waiver is September 17, 2024, through October 16, 2024.

This survey is conducted by Direction Home of Eastern Ohio in partnership with Polco, a trusted research firm and civic engagement organization. The questionnaire, developed by National Research Center at Polco, gathers insight from seniors into overall community livability, covering topics such as employment and volunteer opportunities, housing, accessibility, mobility, and physical and mental health. Results from this survey will help inform the development of local policies, initiatives, and community programs serving older adults.

PASSPORT
An alternative to nursing home care that helps financially eligible, disabled adults aged 60 and over remain safe and independent at home.
Ohio Home Care Waiver
An alternative to nursing home care that helps financially eligible individuals aged 59 and younger, including children, who have significant healthcare needs due to a disabling condition.
Mahoning County Senior Levy
Care management and supportive services for individuals over 60 in Mahoning County who wish to remain at home.
