Direction Home of Eastern Ohio helps older adults and people with disabilities continue to live at home through a variety of free and low-cost resources to keep them happy, healthy and safe.

As a nonprofit organization, we offer long-term services, education and support to Medicaid recipients and qualifying individuals of all ages.

Our Programs

PASSPORT

An alternative to nursing home care that helps financially eligible, disabled adults aged 60 and over to remain safe and independent at home.

Assisted Living Waiver

Provides care for Medicaid-eligible adults in an approved assisted living facility.

 

Ohio Home Care Waiver

An alternative to nursing home care that helps financially eligible individuals aged 59 and younger, including children, who have significant healthcare needs due to a disabling condition.

MyCare Ohio

Comprehensive, coordinated services for residents of Trumbull, Mahoning and Columbiana counties who receive both Medicare and Medicaid services, providing a single point of contact for both systems and simplifying care management.

Mahoning County Senior Levy

Care management and supportive services for individuals over 60 in Mahoning County who wish to remain at home.

Older Americans Act / Title III Programs

Services vary by county but may include home-delivered meals, transportation, and wellness classes.

Specialized Recovery Services

Help for individuals with severe and persistent mental illness, certain diagnosed chronic conditions or those on the solid organ or soft tissue transplant waiting list.

Family Caregiver Support

Support for caregivers of older adults and individuals with a disability, as well as adults 60 or older with custody of someone under 18.

 

Medicare Outreach

Medicare information, presentations and one-on-one assistance from knowledgeable staff.

 

Long-Term Care Consultations

A free, customized in-home consultation that helps individuals understand how to meet current or future long-term needs.

Wellness Programs

Includes chronic disease and diabetes management programs, fall-prevention workshops and wellness presentations.

Senior Farmers Market

Provides access to fresh fruits, vegetables and herbs for older adults living in Columbiana, Mahoning and Trumbull counties.

Senior Olympics

Held annually for people aged 50 and older; events include bowling, tennis, track & field, golf, horseshoes and more. Sound fun? Join us!

 

Long-Term Care Ombudsman

Advocates for those who receive long-term care services in nursing homes, assisted living and group homes, as well as those who receive these services in their own home or community setting, including MyCare Ohio.

Get Out and Dine

The Get Out and Dine program gives Mahoning and Columbiana county residents aged 60 and older the opportunity to enjoy nutritious meals at local participating restaurants while socializing with community members.
