Direction Home of Eastern Ohio is a private, non-profit organization that helps older adults and people with disabilities maintain their health, happiness and independence in the place they want to call ‘home.’

We offer long-term services and support to Medicaid recipients and qualifying individuals of all ages.

With 166,000 clients in Ashtabula, Columbiana, Mahoning and Trumbull counties, our services include:

Free In-Home Assessments to determine eligibility and link clients with community services.

Care Management Representatives to create, coordinate and monitor person-centered care plans and services.

Agency Funding with federal, state and local money to provide direct services to older adults and people with disabilities.

Quality and Monitoring that oversees more than 140 providers.

Family Caregiver Support to offer help and guidance to people caring for a loved one.

Senior Farmers Market to improve access to fresh fruits and vegetables.

Ombudsman Advocacy to protect the rights of individuals receiving long-term care in any setting.

Wellness Programming to support physical, mental and emotional well-being.

Our Story

Direction Home of Eastern Ohio, formerly Area Agency on Aging 11, was created as a result of the Older Americans Act (OAA). The OAA was designed to provide services to older persons not adequately served by other public and private systems. OAA dollars fund many senior services that enable older Americans to remain in their own homes and communities, avoiding unnecessary and costly institutionalization.

More recently, Direction Home of Eastern Ohio has broadened our scope to offer long-term services and supports to Medicaid recipients and qualifying individuals of all ages.

Board meetings are held at 1 p.m. on the 3rd Friday of each month at our offices, 1030 N. Meridian Road, Youngstown.

Our Speakers Bureau offers group presentations on a wide range of topics including caregiver stress management, long-term care choices and Medicare.
