There’s something comforting about remaining in your home, nestled within a community that cares. Through funds generated by the Mahoning County Senior Levy, we are able to offer care management and supportive services to Mahoning County residents aged 60 and older who want to continue living at home.
This program is available to individuals who do not qualify for the Medicaid Waiver.
Eligibility Requirements
The Process
1.
Contact Direction Home of Eastern Ohio to complete a screening over the phone and schedule a free in-home assessment.
2.
We will determine preliminary eligibility and care needs and will provide information about various long-term care options.
3.
If eligible, a dedicated care manager will help develop a customized care plan and coordinate the services you need. *
