Direction Home of Eastern Ohio helps older adults and people with disabilities continue to live at home through a variety of free and low-cost resources to keep them happy, healthy and safe.
As a nonprofit organization, we offer long-term services, education and support to Medicaid recipients and qualifying individuals of all ages.
Our Programs
PASSPORT
An alternative to nursing home care that helps financially eligible, disabled adults aged 60 and over to remain safe and independent at home.
Ohio Home Care Waiver
An alternative to nursing home care that helps financially eligible individuals aged 59 and younger, including children, who have significant healthcare needs due to a disabling condition.
Mahoning County Senior Levy
Care management and supportive services for individuals over 60 in Mahoning County who wish to remain at home.
Specialized Recovery Services
Help for individuals with severe and persistent mental illness, certain diagnosed chronic conditions or those on the solid organ or soft tissue transplant waiting list.
Medicare Outreach
Medicare information, presentations and one-on-one assistance from knowledgeable staff.
Wellness Programs
Includes chronic disease and diabetes management programs, fall-prevention workshops and wellness presentations.
Senior Olympics
Held annually for people aged 50 and older; events include bowling, tennis, track & field, golf, horseshoes and more. Sound fun? Join us!
Kinship Support Program
We want to support Kinship Caregivers! Kinship Caregivers are relatives age 55 or older, who are not the parents and are the primary caregiver, providing care to child(ren) under the age of 18.
