Help for a Better Life

Everyone needs a helping hand when recovering from physical and mental health challenges. Through Specialized Recovery Services, individuals with severe and persistent mental illness, certain diagnosed chronic conditions or those on the solid organ or soft tissue transplant waiting list can get the help they need to achieve a better quality of life.

Eligibility Requirements

21 or older

Diagnosed with a mental illness AND/OR or diagnosed with a chronic condition

Diagnosed with a serious and persistent mental illness AND/OR Actively on the solid or soft transplant waiting list or diagnosed with a chronic condition, including certain malignancies, HIV/AIDS or immune deficiencies, end-stage renal disease, sickle cell anemia, cystic fibrosis, hemophilia or if you have had a previous organ transplant

Need help with medical appointments or essential daily living activities

Do NOT live in a nursing facility, hospital or similar setting

Medicaid-waiver eligible

(low-income household)

The Process

Note: You must be enrolled in Medicaid prior to requesting this waiver. To apply, visit the Ohio Department of Medicaid website.

1.

Complete the waiver request form and submit it to your local county Department of Job and Family Services or contact Direction Home Community Resources to assist with the application process

2.

If eligible, a dedicated care manager will help develop a customized care plan and coordinate the services you need. *

3.

Over time, the care manager will monitor services for quality and make changes to the care plan as necessary.

*If not eligible, we will assist you in finding alternative programs to get you the help you need.

Services May Include:

Assistance in developing a specific care plan

Help with finding and keeping a job

Support from others with similar life experiences

Full Medicaid coverage
